About the Role:
ComForCare Home Care (North San Diego) is looking for a compassionate and skilled Licensed Vocational Nurse (LVN) to join our dedicated home health team in San Diego, CA. Pay: $39-$41 per hour
Location: San Diego County, CA | Travel to clients' homes is required
Schedule: Full-time, part-time, and per diem opportunities available
Responsibilities:
- Provide direct patient care in home settings, including wound care, medication administration, and vital sign monitoring
- Follow and contribute to individualized care plans under the direction of the supervising RN and physician.
- Observe, document, and report changes in patient condition to appropriate clinical staff
- Administer prescribed medications, injections, and treatments per physician orders
- Educate patients and family members on care procedures, medications, and health management
- Maintain accurate and timely clinical documentation in compliance with state and agency standards
- Coordinate with multidisciplinary care teams to ensure continuity and quality of patient care
Requirements:
- Active Licensed Vocational Nurse (LVN) license in the state of California
- Current CPR/BLS certification
- Prior home health, skilled nursing, or clinical care experience preferred
- Strong knowledge of wound care, medication management, and patient assessment
- Excellent communication, time management, and organizational skills
- Reliable transportation and valid California driver's license with proof of insurance
- Compassionate, self-motivated, and able to work independently in a home care environment
